Problem Statement:   As Dengue Virus is spreading very rapidly in Pakistan and it is becoming a serious issue with the passage of time. The Centers for Disease Control (CDC) Pakistan has launched a meeting at the National Institute of Health (NIH), to take new initiatives to enhance the country’s capacity in the field of national public health. The meeting decided to conduct a survey around the different areas of the country so that CDC can be aware of the areas which are getting infected with the Dengue Virus with much rapid speed, so that spray teams can be allocated to those areas. Suppose you have been hired to develop the above-said survey form, which gathers the information of the specific area of people. Sample Survey form:

Solution Instructions: You will use simple color as background.You will use radio buttons and check boxes for selection options (As shown in sample form).Use basic HTML and CSS inline styling where required, which you have learnt till Lecture 8 to design your web page as sample page.You also have to use HTML basic tags, HTML Table, HTML form and tags to design sample web page.   You will make fields to take survey data as mentioned in the sample form. You are required to upload .html file which will have code of HTML. Assignment in any other format (extension) will not be accepted and will be awarded with zero marks.
